Counselling Forms

Before meeting with our mental health team, students MUST fill out and complete the forms below prior to their appointment. You can also send it via email to swc@mcmaster.ca. Failure to complete all necessary forms will delay your appointment.

  1. Complete the Patient Agreement Visit Form.
  2. Complete the Counselling/Mental Health Visit Form.
  3. For International students ONLY, the Cowan Claim form must be completed once per year. 
  4. Register with the SWC clinic by calling 905-525-9140 ext. 27700 or visit the clinic in PGCLL, 210, 2nd floor. You MUST provide a valid health card/UHIP card and student number.
